Even if my screen resolution was more than sufficient to allow the headings to be shown, they sometimes were not shown.
I found out that zooming up in Word was what made the headings in the dialog box disappear even if zooming has nothing to do with the size of the dialog box or the screen resolution. Microsoft is not going to fix this problem.
